A supervisor and employee conduct an annual evaluation that is required for all career employee. Each year your Business Office will provide you with the appropriate forms: Employee Performance Appraisals and Summary of Accomplishment, to complete by a deadline date for submission. You may obtain templates here and examples under Staff Employee Performance and Appraisal Samples in this section.
