Membership fee paid by UC Check

Please fill out completely the Purchasing Order Form http://wineserver.ucdavis.edu/internal/Forms/V&E_Purchase_Request.pdf

and attach the completed application form and submit to Linda’s in box outside of room 1009 Wickson Hall.

A check will be issued directly 1-2 weeks.

Reimbursement for membership fee

If fee was paid by you and after-the-fact reimbursement is requested. A copy of membership application form and proof of expenditure in the form of a receipt or canceled check.

To request reimbursement for membership fee please fill out completely reimbursement request form http://wineserver.ucdavis.edu/internal/Forms/Reimbursement.pdf and scotch tape the receipt(s) to a separate piece of paper and submit to Linda Wiegand.

Please expect it 1-2 weeks for reimbursement.

You can pick up reimbursement checks from Main Office.
